i've had the same problem here but i have an VB6 on my box and so i can open
the Form.
There is another Prob in the Sample. There is fix entry to the Database-Path
D:\usr\ov\databases\odbc\snmpcollect_db.mdb !
I think you should install VB5 / VB6 on your Box if you want check out the
sample.
