Scott,
This should do what you want. I assumed that the Hostname starts with US.
If it does not start with US just eliminate the carat (^).
Note the comparison operator is "bang tilde" meaning "not like".

I want to create a smartset that contains Windows NT 4.0 nodes which is easy

enough, but I want to exclude nodes that have a naming convention of 
USXXXWXX (The X being any number or letter). Is this possible?
